Important info about completing our wellness program

Ballad Health is a health improvement organization, and we’re starting with ourselves. We want our team members (and their families) to lead healthy, active lives, so we’re encouraging team members to participate in our wellness program, available for those who chose the Ballad Health Choice Plan as their health insurance.

Most of you know about the significant discounts on your insurance premiums if you complete the wellness program, but the program is about more than that. It’s intended to help you maintain your health, or where needed, provide programs and services to help you address specific health risks that have been identified.

Here are some important details and updates:

 

If you haven’t done your screening, there’s still time

There’s still time to do your health screening. There are three makeup dates left in August:

  • Wednesday, Aug. 8 – Lonesome Pine Hospital, classroom, 2-8 p.m.
  • Saturday, Aug. 11 – Indian Path Medical Center, conference center, 8:30 a.m.-2 p.m.
  • Monday, Aug. 13 – Smyth County Community Hospital, administrative conference room, 3-8 p.m.

If you complete your screening during one of these makeup sessions, your results will be available after Aug. 24.

If you still need to complete the personal health survey, do so as soon as possible. You can access the team member survey and the covered spouse survey by going to the Ballad Health intranet here.

 

If you’ve already completed your screening, get your results

Team member final results and your personal health report will be available after Aug. 13 if you completed your screening by July 26, 2018.

As part of this program, you’ll receive a personalized Personal Health Report which details your current health profile and provides helpful advice on how to begin improving your health. You may already have received an email about this.

The health portal allows you to view and download your current and past Personal Health Reports, find late breaking health articles, get a snapshot of your health trends, communicate with a health coach or clinician and participate in personalized health offers from respected local and national partners.

What’s next: Health coach will contact you

After you receive your personalized report, a health coach will reach out to you by telephone to discuss the results of the report with you and/or your spouse. Your risk factors will determine the number of required sessions with your health coach.

It is a requirement to speak to one of the health coaches when they contact you. If you’re unable to take the call at that time, the health coach will leave a contact number for a return call. Return the call as soon as possible in order to maintain the health premium discount.

 

Your health

In some cases, people have more significant health conditions that need to be addressed. The wellness program is intended to support, not replace, the relationship and plan of care that you may already have in place with your physician.

In all cases, your personal health information will be handled with the utmost confidentiality.

 

You’ll get an email about your portal

You’ll receive an email about accessing your portal, with a subject line of “Welcome to Your Personal Health Portal.” This is a legitimate and safe email, not a phishing email (see below for what this email looks like), but if you’ve already received it and weren’t sure whether it was legitimate or not – thank you for being aware and suspicious! Email security is EXTREMELY important for us.

This email will come from Ballad Health but the return email address is mailer@personalhealthsurvey.net.
