How to sign up for MyChart
You can sign up for MyChart through the Ballad Health app. (NOTE: This is a separate app from the Ballad Teams app.) Download the Ballad Health app by following the instructions matching your device:
For iPhone and iPad users:
- Go to the App Store
- At the bottom, click Search
- In the search bar, type “ballad health”
- Look for the Ballad Health icon
- Follow the directions to download the Ballad Health app
- Open the app and select “create account”
For Google Android phone and tablet users:
- Go to Google Play
- Go to the Apps section
- In the search bar, type “ballad health”
- Look for the Ballad Health icon
- Follow the directions to download the Ballad Health app
- Open the app and select “create account”
If you already have a registered MyChart account, you’re already on your way to winning! If you’re selected for a prize, we’ll reach out to you. This contest will run through June 28, but the earlier you get your account set up, the more opportunities you’ll have to be entered.
Full Contest Rules
This sweepstakes (“Sweepstakes”) is organized by Ballad Health (“Sponsor”), with its principal place of business as indicated on its official website. The Sweepstakes is governed by these official rules (“Official Rules”). By participating in the Sweepstakes, each entrant agrees to abide by these Official Rules, including all eligibility requirements, and understands that the results of the Sweepstakes, as determined by Sponsor and its agents, are final in all respects. This Sweepstakes is not sponsored, endorsed or administered by, or associated with any third-party entity.
Eligibility
The Sweepstakes is open only to Ballad Health patients who are 18 years of age or older at the time of entry. Whether you’re enrolling in MyChart for the first time during this period or you’re already a registered user, you’re eligible to participate.
Sweepstakes period
The Sweepstakes starts on Feb. 5, 2024, and ends on June 28, 2024. The first drawing will take place on Feb. 16, 2024, with a drawing happening each Friday after that through Friday, June 28, 2024. All entries must be received on or before the time stated during that submission period. This Sweepstakes is available only to US residents and excludes residents of NY, FL, and RI.
How to enter
Entrants must activate their MyChart account or already have a MyChart account, during the Sweepstakes period, to be eligible for each drawing. Each MyChart account, with patients over the age of 18, is eligible for one entry into the drawing. If you are a Ballad Health patient, you will automatically be entered into the Sweepstakes once you have activated your Ballad Health MyChart account.
Winner selection
A winner will be chosen by a random drawing each week starting Feb. 16. The winners will be notified directly.
Prizes
The winners will receive a $500 Visa gift card. The gift card may be used like a regular debit card but cannot be redeemed for cash or reloaded. The odds of winning depend on the number of eligible entries received in addition to the current number of active Ballad Health MyChart accounts. The greater the number of eligible entries, the greater the odds that you might win.
Additional limitations
If you win this promotion, you will not be eligible for other Ballad Health promotions or giveaways for the remainder of 2024. Sponsor is not responsible for any typographical or other errors in the offer or administration of the Sweepstakes, including, but not limited to, errors in any printing or posting of these Official Rules, the selection and announcement of any winner, or the distribution of any prize.
Indemnification and limitation of liability
By entering the Sweepstakes, each entrant agrees to indemnify, release and hold harmless Sponsor and its parent, affiliate and subsidiary companies, administrator, advertising and promotional agencies, and all their respective officers, directors, employees, representatives and agents from any liability, damages, losses or injury resulting in whole or in part, directly or indirectly, from that entrant’s participation in the Sweepstakes and the acceptance, use or misuse of any prize that may be won.
