Dear Ballad Health team member,

It’s that time of year – annual benefit open enrollment for all benefit-eligible Ballad Health team members! This is a great opportunity to review your current benefits, make changes, learn about new benefit offerings and update your beneficiary information. Open enrollment is underway and runs through May 29. Even if you do not need to make changes to your benefits, some benefits require an election. You can enroll by logging into the HR & Payroll Service Center. Click here for enrollment instructions and the detailed benefit guide.

Details about the FY20 benefits package:

  • Medical/Wellness Program – We will continue to offer two high-deductible health plans through BlueCross BlueShield of Tennessee – the Ballad Health Choice plan and the Ballad Health Bronze plan. While there will be a slight increase, team members’ cost share of the premiums remain significantly below market. The lowest wellness/non-tobacco premium rates will show during the online open enrollment process. To continue the lowest premiums, you and your spouse (if applicable) are required to complete the two-part wellness screening and be non-nicotine users. The wellness screenings are available at various locations through June 5. Click here to review the premium rate chart. Rate adjustments will reflect in the July 12 pay date.

 

  • Employee Assistance Program (EAP) – All benefit-eligible team members and covered dependents are eligible for EAP at no cost. Services are expanding to additional locations.

 

  • Health Savings Account – For team members enrolled in the Choice plan, Ballad Health will continue to make a $600 contribution for team member-only coverage and a $1,200 contribution for team member and children and family coverage to your HSA. Contributions are made half in July and half in January. If you are currently contributing to the HSA, you will need to make a new election during open enrollment.

 

  • Diabetes Program – The diabetes management program is available to team members and dependents enrolled in one of the medical plans who have a diagnosis of diabetes. Participation is free.

 

  • Pharmacy – No change. Please review the updated drug listings online for July 1.

 

  • Dental & Vision – No change in benefit or premium.

 

  • Supplemental Life Insurance – Team members not currently enrolled in supplemental life coverage can elect 1x salary in coverage without completion of an evidence of insurability form (EOI). Team members currently enrolled in coverage can increase 1x salary in coverage without EOI. Additional life coverage requested will require EOI.

 

  • Dependent Spouse Life & Child Life Insurance – Team members not currently enrolled in dependent spouse coverage can elect $10,000 in coverage without completion of an EOI form. Team members currently enrolled in coverage can increase one level ($10,000) in coverage without EOI. Additional spouse life coverage requested will require EOI. Child life coverage in any amount can be elected without EOI.

 

  • Short-Term Disability – Short-term disability can be elected or increased without completion of EOI during this enrollment period. New or increased coverage elections are subject to pre-existing conditions. Beginning July 1, team members will not be required to deplete their MML balance prior to being eligible to receive short-term disability payment. Short-term disability will be paid after the waiting period.

 

  • Long-Term Disability – Team members can elect to increase coverage without completion of EOI during this enrollment period. The increased coverage election is subject to pre-existing conditions.

 

  • Flexible Spending Accounts – To continue the medical and/or dependent care spending account, a new election must be made during this open enrollment period. Note: If you are enrolled in the HSA you are not eligible for the flexible medical spending account.

 

  • Voluntary Benefits – We’ve worked hard to standardize the voluntary benefit offerings for Ballad Health team members. During this year’s enrollment, team members will have the opportunity to enroll in a selection of new voluntary benefits with enhanced services at comparable or reduced rates. You will need to go online and enroll or decline the plans. Team members can enroll in the following plans on the HR & Service Center Portal in Lawson: critical illness/accident, legal insurance and identity theft insurance. After completion of the open enrollment process you will be prompted to enroll in the whole life/long-term care plan. Team members will also be able to enroll in pet insurance, home/auto and purchasing power outside of the HR & Service Center Portal by clicking on the company links. All voluntary plan information is located on the Open Enrollment link on the Ballad Health Benefits page. Important: If you are enrolled in an existing voluntary benefit plan, payroll deduction will end as of June 28. You will either need to enroll in one of the new plans or make arrangements with the current vendor to continue coverage by setting up direct payment. Additional communication will be sent to team members currently enrolled in a voluntary plan. Need help?   • AirMedCare – Team members will be able to enroll in the Air Flight program through AirMedCare using their online portal. Payroll deduction is available over one pay period for an annual or multi-year membership plan.

 

This is a lot of information so there are multiple resources available to you.
