Wellness survey now open: Here’s what you need to know about the survey, screenings, health portal

Our Ballad Health wellness screenings are underway and the wellness survey is now available effective Monday, April 1.

Here are some of the key links for participating in the wellness program:

Reminder: Both the team member and covered spouse must complete the wellness components to be eligible for the premium reduction and additional wellness programs, such as Diabetes Management Program.

Survey FAQs

When will the wellness plan survey be available?

  • The Ballad Health wellness plan survey is open effective Monday, April 1, 2019, and will conclude on June 5, 2019.

How do I access the survey from a Ballad computer?

How do I access it from a personal computer?

  • The survey can be accessed from any computer or smart device at your leisure. You do not have to complete the survey on a Ballad Health computer or through a remote desktop access. The survey can be accessed at the following link: https://ballad.ezonlineregistration.net/.

What information do I need to complete the survey?

  • In order to begin the survey, you will need to have your Ballad Health team member ID number and birth date. (Your badge number is not the same as your team member ID number)
  • If you are a spouse, you will need to use your employed spouse’s team member ID number followed by an “s”. (Example: 98765s)

Where do I locate my team member ID#?

  • All team members will receive an email from HR4U on Friday, April 5, that will include your Ballad Health team member ID number.

The system does not recognize my team member ID number or birthdate. What should I do?

  • If you cannot access the wellness plan survey due to an incorrect team member ID number or birthdate, please call 855-581-9910.

What if my spouse and I both work at Ballad Health?

  • If you and your spouse are both employed by Ballad Health, use the team member ID number of the spouse who is listed as the primary on the coverage plan.

What if my information in the survey is incorrect or I would like to change my contact information?

  • Contact information may be updated at the time of your survey or afterward by logging into your Health Portal. It is very important that you keep your contact information up-to-date.

Screening FAQs

  • When will the screenings be held?
    • Wellness screenings began on March 18, 2019, and will conclude on June 5, 2019.
  • Do I have to screen at the facility where I work?
    • You do not have to complete your wellness screening at your facility.
  • Can I bring my labs/biometrics to the screenings?
    • Yes, labs and biometrics can be presented at any screening through June 5, 2019. Labs and biometrics can be faxed (423-230-8416) or emailed (TMwellness@balladhealth.org). Please include a good contact number with the fax or email. Receipt confirmation or notification of incomplete labs/biometrics will be provided. Faxed or emailed results will be accepted through May 15, 2019.
  • Where can I find a list of the required biometric and lab values? Is there a specific form I must use to submit my values?
    • The biometrics and labs required: Height, weight, blood pressure, heart rate, total cholesterol, HDL, A1C.
    • Lab and biometric result may be submitted in the format received from your provider. A provider form has been created for your convenience, although documentation in any format can be submitted.
    • You can find the provider form on the Ballad Health wellness page.
  • Can I make an appointment?
    • You cannot make an appointment for your wellness screening.
  • How long will the screening take?
    • Your wellness screening should not take more than 5-10 minutes. This does not account for wait times.
  • What does the screening entail?
    • The screening includes obtaining height, weight, blood pressure, heart rate and blood for a lipid panel and Hgb A1C.
  • I am on FMLA during the screenings. Can I still screen?
    • Yes, you can still screen. If you are unable to come to a screening event, you must complete all components of wellness (survey, biometrics, and labs) within 30 days of returning to work. You can complete your labs and biometrics at 1004 Executive Park Blvd., Suite 3, Kingsport. If you have a spouse on your insurance, the same guidelines apply.
  • My spouse is unable to attend the screenings related to physical limitations. Is there an alternative?
    • If your spouse is unable to complete any component of the wellness requirements (including coaching sessions), your spouse will need to complete a Medical Exemption Form and return it to the wellness department. The form can be found here on the Ballad Health wellness page and faxed to 423-230-8179.
  • I am a new hire. Do I need to complete the screening?
    • Team members hired after May 1, 2019, are not required to complete a wellness screening. If you would like to participate in the wellness program and are hired after May 1, 2019, please contact the wellness department for directions.
  • Am I required to fast for labs?
    • You are not required to fast for labs. Additional cholesterol lab results can be provided if you elect to fast prior to your screening.
  • Who is required to complete the screenings? (Participation in the wellness program is voluntary. However, if you wish to receive the benefit of decreased premiums or to participate in the Diabetes Management Program, then you must participate.)
    • Team members electing the Choice Health Plan
    • Spouses of team members in a Choice Health Plan
    • Team members and spouses in the Diabetes Management Program
    • Team members in union plans who wish to participate in the Diabetes Management Program
  • I am or my spouse is out of the country?
    • You may fax your labs/biometrics (required form) dated March 1, 2019, or after. Fax to 423-230-8179. Forms must be received by June 5, 2019.
  • I just had a physical with my provider. Do I still need to complete the screening?
    • Team members and covered spouses can submit lab results and biometrics from an outside provider on or after March 1, 2019. Lab and/or biometrics may be submitted by email or fax through May 15, 2019, or presented to any screening through June 5, 2019. If it contains all of the required components (lab results and biometrics), then you do not have to complete a wellness screening. It is your responsibility to get your information to the wellness team.
  • Can I fax my labs and biometric data?
    • You may fax your labs and biometric data (required form) to the wellness department at 423-230-8179. Please include a contact number or email.
  • Do I need to complete the wellness plan survey prior to the wellness screening?
    • You do not have to complete the wellness plan survey prior to the wellness screening. Your survey must be completed prior to June 5, 2019.
  • Will there be additional makeup dates?
    • There will be no additional wellness screening dates.

Portal FAQs

  • How do I create a personal health portal account?
    1. Go to https://balladhealth.personalhealthportal.net/login.
    2. Click “Create Account.”
    3. Enter the requested information and click “Create Account.” Your information must match the information you provided in your wellness plan survey.
    4. Create a username and password.
  • I don’t remember my username or password.
    1. Go to https://balladhealth.personalhealthportal.net/login.
    2. Click “Forgot Username” or “Forgot Password.”
    3. Enter the requested information and click “Reset Password” or “Send Reminder.” Your information must match the information you provided in your wellness plan survey.
  • When will my results be available?
    • Your personalized health report will be available at the beginning of the new plan year (July 1). In order to access your personalized health report, you must complete the wellness plan survey and fulfill your biometric and lab obligations.
  • What information will be available in the portal?
    • Your health and lab reports
    • Trends in your lab and biometric results
    • Your personal goals
    • Videos about health risks, such as blood pressure, depression, sleep apnea, etc.

Post-Screening FAQs

  • When will I be contacted by a wellness coach?
    • Wellness coaching will begin shortly after the start of the new fiscal year.
  • I am a tobacco/nicotine user. When will the cessation education classes be offered?
    • Classes will begin Aug. 12, 2019.
  • Who has access to my information and results?
    • You

    • Your PCP and healthcare team
  • Will I receive emails outside of the wellness team if I provide my email address?
    • No, you will only receive emails from the Ballad Health wellness team.
  • How will I be contacted?
    • The Ballad Health wellness team could contact you via email or phone.
  • I haven’t been contacted by a health coach, but would like to speak with someone about my results?
    • If you would like to speak with a wellness coach, please contact the wellness office at 1-888-314-7803.
