Scheduled health screenings now over

If you’re participating in the Ballad Health wellness program and you still need to get a health screening, there are only a few opportunities remaining. The last scheduled screening was June 5, and there will be no makeup dates scheduled.

Remember: If your spouse is on the Ballad Health insurance plan, they also need to complete a wellness screening in order to get lower premiums.

SUBMITTING A PRIOR HEALTH SCREENING

The health screenings will include a lab work (blood draw), as well as measurements for blood pressure, heart rate height and weight. There is no fee for receiving a screening and no appointment is needed.

  • If you have lab work that includes a cholesterol panel and an A1C done by your own doctor or provider on or after March 1, 2019, you can substitute that for having your lab work done at one of the scheduled screenings.
  • You can also submit your biometrics (height, weight, blood pressure and heart rate) that were completed by your doctor or provider along with your labs, if completed on or after March 1, 2019.
  • Results may be brought to any screening until June 5, 2019, for validation.
