Here’s what you need to know about our Ballad Health wellness program

The team member wellness program is an important part of Ballad Health’s benefits package. If you select the Ballad Health Choice Plan for your medical insurance, you can get a significant savings on your premiums by participating in our voluntary wellness plan. But more importantly, it can lead to better health – and a happier life – for you!

This fits with Ballad Health’s mission to improve the health of our region, and it makes sense to start with ourselves. The wellness program encourages making healthy lifestyle choices and rewards team members who follow through with the plan. It’s simple and we’ve tried to make it as convenient as possible for all team members.

 

HOW DOES IT WORK?

If you picked the Choice Plan, how much you’ll pay in insurance premiums will be determined by whether or not you do the wellness program, and whether or not you use tobacco. If you don’t use tobacco and you do complete the wellness program, you’ll get the lowest premium rate. Click here to see a chart of the premiums for the different categories and rates.

As part of the wellness program, health screenings will be offered in June and July, and if you complete your screening and a personal health survey by July 26, 2018, you’ll receive a discounted rate on your premium (also reflecting tobacco usage).

NOTE: Everyone on the Choice Plan will automatically start out the new fiscal year (July 1, 2018) paying the lowest premium rate for your medical insurance. Then after July 26, that premium rate will be adjusted according to wellness program participation and tobacco usage.

 

WHAT’S INCLUDED IN THE WELLNESS PROGRAM?

Here’s what you’ll need to do:

1, Take an online personal health survey. Here are links to the team member survey and to the covered spouse survey.

2, Get your lab work and biometrics done. Free screenings are scheduled in all four of our Ballad Health markets at various facilities so you can conveniently complete them. See accompanying wellness program screening schedule posted below, or access it on the Benefits page of the intranet here.

3, Go online to get your screening results.

4, Talk on the phone to a health coach about your results.

As long as you complete the personal health survey and get your health screening by July 26, you’ll receive a discounted rate for your premium. Once you get your results, you’ll be expected to follow up with your health coach for a phone session, but that can be completed after the July 26 deadline. A FEW DETAILS ON EACH PART OF THE PROGRAM

Online personal health survey – This online survey is available starting May 31 on the Benefits page of the Ballad Health intranet. It assesses lifestyle habits and medical history. The results you provide will be strictly confidential and are intended only as a resource to help you achieve better health by focusing on behaviors and habits you can change. No one except the wellness coaches – including human resources, your manager or other team members – will have access to your answers.

Lab work and biometrics – Includes height, weight, blood pressure and blood work. The screening takes about 15 minutes. Check the wellness program screening schedule above, or on the intranet, to see when they’ll be offered in your market. These have been scheduled throughout the system so you don’t have to make an appointment or travel to have your screening. But if you can’t make it to a screening at your facility, you’re welcome to go to any other scheduled screening at another facility. Details will be provided on where to access your results online. NOTE: If you’ve had lab work done in the last 90 days that includes a lipid panel (total cholesterol, HDL) and hemoglobin A1C, you can bring a copy to your screening and you won’t need to do blood work.

Health coaching – After you get your personalized report, a health coach will reach out to you by telephone to discuss the results of the report with you and/or your spouse. Your risk factors will determine the number of required sessions with your health coach. Every participating employee and spouse is required to speak to a health coach a minimum of one time. Team members identified with two or more health risks are required to speak to their health coach three times per year. These coaching sessions are required to maintain the health premium discount.

 

SPOUSAL COVERAGE

If you are eligible and elect to cover your spouse under the Choice Plan, your spouse must also participate in the free wellness program to receive a discounted health premium. Team members and their eligible spouses covered under the health plan who participate in this program will receive a discount on the premiums for health coverage. Team members and/or spouses who use nicotine, or do not fully participate in the wellness program, will pay a higher premium for health coverage.

 

DIABETES DISEASE MANAGEMENT (DDM)

Team members who are participating, or if their spouse or dependent is participating in the DDM program, are required to complete all components of the wellness screening. Spouses who are participating in the DDM program are also required to complete all components of the wellness screening.
